Shock is a clinical syndrome that is a result of inadequate tissue perfusion that creates imbalance between the delivery of and requirements for oxygen that support cell health.  Signs and symptoms of excess fluid volume or inadequate tissue perfusion characterize heart failure.

  1. Describe the differences in the nursing management for patient diagnosed with hypovolemic shock, cardiogenic shock, and heart failure.
  2. Because heart failure can be a chronic condition, identify a priority teaching intervention for the patient with a new diagnosis of heart failure.

good intervention for an immunosuppressed patient

A good intervention for an immunosuppressed patient is an incentive spirometer.  My grandmother would use an incentive spirometer for her COPD.  It wasn’t always easy to have her use the spirometer.  In addition, I have had to teach myself to use it, when I was diagnosed with pneumonia and had a very bad cough years ago.  It’s quite difficult to use when you are feeling ill and all choked up.
Some effective obejctives for a spirometer as a nurse include:
1. Teach patient of benefits of  Incentive spirometer
2. Teach patient that the reason for incentive spirometry is to help him fully inflate his lungs
3. Patient will report number of times used daily and mark progress with indicator
4. Patient will demonstrate knowledge of COPD process, meds, therapy and follow up care.

Pulmonary Mucormycosis

Pulmonary Mucormycosis is most common in patients that are immunocompromised. Early diagnosis for early treatment is essential for this life-threatening disease. Yes, depending on the patient’s reason for having a low immunity, neutropenia can occur and increase the patient’s risk for infection even greater. But, the patient in our DQ had a high WBC count. The lab did not show differentiation of the WBCs. Doesn’t the elevated WBC count mean the person’s immune system is trying to fight the fungus? I do not think our patient needs colony stimulating factors for neutropenia. But our patient may need surgery as you pointed out. “There are four determining factor in the successful treatment of mucormycosis: early diagnosis, amphotericin B, surgical resection and reversal of host impairment” ( Oluwatobi, et al., 2016).

Asalah is a Muslim female who has immigrated to the United States with her husband and two children. Her husband is considered moderate and progressive in his ideals. Asalah is working in the female section of the county jail. Although she is progressive, she still feels that she should wear a khimar (type of head scarf), but by policy, this is not allowed within a jail setting.
To follow policy and procedure, she does not wear any covering in the female section but wishes to wear the khimar when coming into and leaving the jail setting because there are males in this institution.
She has just heard from her family that they are coming for a visit. Her family is quite conservative and also realizes that she will not be wearing a burqa but would expect her to wear a khimar.

  • Could there be an exception made for her to come to and from work?
  • Knowing that she is Muslim, would this head scarf put her in any danger from the female inmates?
  • The jail has offered to place her in the classification department if she wishes to wear the khimar, but would this be considered discrimination? Why or why not?
  • During her lunch hour, she wishes to go to the cafeteria to get some snacks but she is not allowed to wear her khimar. This causes a problem because men are walking the halls and are in the cafeteria. What would be the reasoning that the jail administrators could give her for this procedure?
  • What types of policies exist regarding cultural considerations such as this? Explain.

  • Regents v Bakke – Supreme Court Case
    Regents v. Bakke – This unit’s DB asked that you explore the issue of the security of prohibiting the wearing of a hijab. Please include in your discussion this court case and its allegations that by showing favor to one you violate the rights of others. Case deals with California’s implementation of affirmative actions programs at the University of California, Davis.

Thomas Farrow had been evaluated as having been inflicted by managerial hubris at the time of the bank’s collapse in 1920. With this in mind, address the following questions, with thorough explanations and well-supported rationale.

Jane Addams and Mary Richmond
Jane Addams was a well-known social worker for her work in social policy Addams founded the Hull House in 1969 (Jansson, 2018). Addams work assisted individuals who suffered from poverty and oppression.
Mary Richmond also contributed to social work through writing books and providing educational information that she provided to charities and related agencies. Richmond’s work provided advocacy in building healthy relationships.
Both are great contributors in social work however have different ideas and interests in serving the community.
Schisms 
The Code of Ethics provided by the NASW sets standards for all social workers to follow and abide by. According to NASW (2008), “the Code of Ethics is intended to serve as a guide to the everyday professional conduct of social workers” (para. 1). Schisms still exists in the world today, but perhaps social workers are working together for the greater good. Social work is split into the micro, mezzo and macro level systems. Each group has a set of responsibilities and advocacy practices that best pertains to their system. The Code of Ethics is great way to ensure that social workers are fulfilling their ethical commitments to clients, especially because the Code of Ethics is universal amongst all social workers.
