Use of correct English, spelling and grammar is a must. Sources must be cited in APA format. Your response should be a minimum of four (4) double-spaced pages. Each part should be at a minimum 2 pages long.
Part A: Kayla Christianson, CMA, has been employed six years by the cardiology practice of three physicians. She is a graduate of a CAAHEP-accredited school. Furthermore, Kayla received extensive hands-on training performing ECGs while doing her required externship.
Kayla has completed an ECG ordered by Dr. Hsu for Mrs. Warner, a 76-year-old patient. Dr. Hsu, Kayla’s boss, has telephoned her explaining that he was behind schedule doing rounds at the hospital. He asked her to do him a favor and interpret Mrs. Warner’s ECG, sign his name, and fax the report to Mrs. Warner’s referring internist who is expecting the results.
Provide detailed answers for each of the following questions.
Your response should be two (2) pages in length.
· Given the scope of Kayla’s education, training, and years of experience as a CMA, would this favor fall within the AAMA guidelines of her responsibilities?
· Would any portion of Dr. Hsu’s request fall within the guidelines? If so, which portion(s)? Is an exception to these guidelines ever allowed?
· How should Kayla respond to Dr. Hsu?
Part B: For more than two years, medical assistant Linda Lewis had been employed by Drs. Norek and Klein, who are gerontologists. Also on staff are two registered nurses, a medical laboratory technician, and a medical social worker. The daughter of one of the doctor’s patients has just called the office. She is very distraught at the seemingly diminished capacity of her mother and insists on speaking to the doctor.
Linda explains that both physicians take only emergency calls during patient appointment hours, but that she will take a detailed message. This caller, however, suggests that not only should her call be considered an emergency, but that she will sue the doctor if the call is not handled accordingly.
Response should be two (2) pages in length.
· What should Linda do immediately to diffuse the situation?
· Is this clearly a case when the call should be passed on to one of the registered nurses or the medical social worker?
· Is this a case when the physician should be called to the telephone because of the threat of an impending suit?
· How could Linda ascertain whether or not this is indeed an emergency? Is it up to her, as a medical assistant, to make such a determination?
· Because this is the patient’s daughter rather than the patient herself, does Linda have any reason to enter into a conversation with the caller? Could Linda be ethically bound by confidentiality not to admit the woman’s mother is a patient?

Adolescent pregnancy
Adolescent pregnancy is viewed as a high-risk situation due to the serious health risks that this creates for the mother, the baby, and society at large. Describe various risk factors or precursors to adolescent pregnancy. Research community and state resources devoted in adolescent pregnancy and describe at least two of these resources. Research the teen pregnancy rates for the last 10 years for your state and community. Has this rate increased or decreased? Discuss possible reasons for an increase or decrease.
Suicide
Suicide is the third leading cause of death for ages 15-24. Adolescents is a stressful time of transition and when adolescents do not have good coping mechanisms in place, or do not have people they trust to confide in about personal things they may feel that suicide is the only option they have (Edelman, 2014). It is important for the nurse to assess for any warning signs of suicide during the patient interview. If the patient has a history of prior suicidal attempts, feelings of hopelessness or depression, isolation, sleep disturbance, history of suicide in the family, physical or sexual abuse, self-mutilation, eating disorder, risk taking behavior, and access to firearms requires further investigation. The nurse should also assess if the patient is preparing for death, such as giving personal valuables away, or voicing statements of failure, tired of trying, and low self-esteem. Patients with history of anti-depressants, alcohol and substance abuse are more prone to suicide and should be monitored closely (Jarvis, 2016).
Barriers that might hold nursing practice from achieving 90% of practices
Barriers that might hold nursing practice from achieving 90% of practice be evidence-based by 2020 include priority and education towards the practice itself. A research done by a group in Columbus, Ohio surveyed 276 chief nurse executives to see how evidence based practice rank in their institutions. The studies states that even though it is known that evidence based practice has resulted in high quality care and improve population health as well as showing better patient experience and lowering costs it is not a priority in most institutions. (Nurse.com)
The priority focus in most institution that are not practicing evidence-base practice is not evidence-based practice but other issues such as quality of care and safety. The mind set is that as soon as they have a handle on quality of care and safety then the next issue will be evidence-based practice. I think institutions are thinking that to compile evidence on issues and compare them is too time consuming so they rather to focus on other issues. An example used in the article is that Emergency Department continue to treat children that present with asthma with nebulizers when there are numerous studies that show that the outcome is better when a metered bronchodilator is used. “when patient get, evidence based care, they have 28% better outcome. (nurse.com)
Most of the chief nurses that were in the survey was unsure of how to measure patient outcome. This is where education comes in. Nurses are graduating with doctoral degrees. There curriculum needs to emphasize the importance of evidence-base practice and how its use can facilitate better outcome. In this way when these nurses are practicing they will be more inclined to rely on evidence-base practice.
politics and organizational cultures
Even though there is growing awareness of the importance of evidence-based practice, in education there is a reported gap between what has been found by research to be effective and what educators use and practice in their daily work. “A new national survey of more than 1,000 RNs suggests that resistance from nursing leaders and other barriers prevent nurses from implementing evidence-based practices that improve patient outcomes. When survey respondents ranked these barriers, the most frequently mentioned also included politics and organizational cultures that avoid change.” (Nurse.com, 2012) When asked what would help them implement evidence-based practice, respondents reported education, access to information and organizational support among their top needs.
Another barrier can be policies in certain agencies. Although agencies often report that they support evidence based practice, when it comes to making even the best evidence-supported changes in policy or procedures, recommendations can and do still meet with resistance. Commonly identified individual barriers reported include lack of knowledge about research methods, poor awareness of research findings, and negative attitude towards having to do research which then leads to not having a certain policy or procedure in place.
When looking at the challenges that come up in the process of implementing evidence based practice in areas of work many things can be prevented by addressing the issues at hand and start using more evidence based practice. By addressing these dilemmas, challenges can be minimized, although not totally avoided or eliminated. By creating the ability to commit necessary resources to implement EBP approach in its entirety, including providing relevant and ongoing training for all workers.
Informatics in the Clinical Setting
Informatics in the Clinical Setting Conduct an interview with someone in an informatics role in a clinical setting. Then, compose a 4-5 pages paper discussing the following components:
•Role description and education level required for the role.
•Describe their views on how their role impacts patient safety and improves the quality of patient care.
•Describe the Human Factors faced in the role and other challenges.
•Express insights gained from the interview.
•Research and discuss the impact of the Agency for Healthcare Research and Quality (AHRQ) Patient Safety Indicators and The Joint Commission Patient Safety Goals in your clinical setting. Describe how these governing agencies impact delivery of direct patient care.
•Research, discuss, and identify the Technology Informatics Guiding Education Reform (TIGER) utilized in your clinical setting. Be sure to get the perspective of your selected informatics professional during your interview.
•On a separate page, cite all sources using APA format.
interpretation of a literary element
By the due date assigned, attach the rough draft of your essay as a Microsoft Word document to the Discussion Area. By the end of the week, comment on at least two of your classmate’s submissions using the peer review questions provided below.
IMPORTANT NOTE: Your instructor will assign 3-5 short stories for the class to read and discuss this week. Check the Week 4 Announcement for a list of the assigned stories. You must choose one of these stories to earn credit on the assignment.
Compose an analytical essay of at least 1,200 words in which you offer an interpretation of a literary element in one of the assigned short stories. Write your analysis focusing on one of the following elements in one of the assigned stories:
- Character
- Theme
- Symbolism
- Imagery
- Setting
Start by selecting one of the short stories assigned by your instructor. Brainstorm to identify the literary element that you would like to explore in the story. Choose from character, theme, symbolism, imagery, or setting. Then, develop a thesis that offers a specific interpretation of this element. If you have trouble coming up with a thesis, contact your instructor, who will help you. Do not do any outside research at this point. When finished, the draft should be at least 1,200 words (approximately four double-spaced pages).
Tips for the Essay
- Open your introduction with an engaging opener, such as a question, quote from the story, or interesting idea. Then, connect to the short story and mention the title and the author. End your introduction with a thesis statement that interprets one literary element of the story (such as theme, symbolism, or characterization).
- The body paragraphs should support your thesis. Present specific aspects of the short story that help to illustrate your points. Make sure to quote from the story and analyze specific lines that support your argument.
- Include a strong concluding paragraph that summarizes your main points and explains the significance of the thesis.
APA Reminder
Use APA style for formatting the essay and for source citations. Begin with a title page. Include a running header, and use proper font and spacing. End with a separate references page. Refer to the Week 1 lecture on avoiding plagiarism for an APA essay template and additional resources.

Public Health Epidemiology

Public Health Epidemiology 1 pg Discussion APA W/2 REFERENCES
TOPIC Assessing Causality and Implications for Public Health Policy
In 1965, Sir Austin Bradford Hill proposed guidelines for assessing causality from epidemiological studies. His intention was not for these guidelines to be used in a rigid manner but rather to be used generally to help in the assessment of disease causation. In recent years, other causal models have been developed, including the Rothman sufficient-component cause model and the web of causation.
To prepare for this Discussion, use the Walden library to find an epidemiological research article published in the last 3 months that uses one of the models of causation discussed in the Learning Resources. Make sure the epidemiological research article you choose clearly uses a model of causal inference that you can discuss. Not all articles do this, so choose carefully! The best options are review articles or articles that clearly address an epidemiological causal inference model in their title and/or abstract.
Respond to the following questions:
What is the epidemiological causal inference model used by the authors? How did they apply the model to the association they studied?
Do you agree with the author’s choice of causal model? Why or why not? Provide evidence to support your assessment.
What are the implications for public health policy, given the evidence for causality presented in the article?
Make sure that your discussion focuses on evaluating your article using the entire causal model that the authors used (as opposed to using one or two criteria). Be sure to cite the article you reviewed using proper APA formatting, and include in-text citations and references as appropriate.
