1. Review the details of the Deepwater Horizon disaster on the Wikipedia article titled “Deepwater Horizon explosion” (http://en.wikipedia.org/wiki/Deepwater_Horizon_explosion).
a) Many warning signs and incidents occurred before the explosion, but engineers and management chose to ignore them. List three of these near-misses, and explain why each should have been viewed as a warning of the impending disaster (i.e. what specifically did they suggest might go badly wrong?)
b) Why were operational problems and concerns about safety not reported to (or acted on) by the upper management who were ultimately responsible for the Deepwater Horizon?
2. Read the article titled “Blowup” by Malcolm Gladwell (posted on Canvas).
a) Explain what is meant by the term “normal accident”.
b) Why does our ever-growing reliance on complex safety systems in many aspects of our lives (e.g. antilock braking systems, network firewalls, smoke detectors, etc.) actually encourage people to take more risks than they would have otherwise?

a) How was Garcia killed? What was the official cause of death? Who was ultimately responsible for Garcia’s death?
b) List at least four general behaviors or actions that Garcia took, which violated accepted industry safe practices for work in confined spaces. (do not simply list four missing items of PPE, etc.)
c) Explain what was meant by “near-miss confined space fatality” in the article.
d) What is the main subject of the legal dispute between Garcia’s family and Reynolds?
4. Consider the process flowsheet above. In this process, hot gasoline containing methane gas is pumped by a positive-displacement pump (P-1) into a separator drum (D-1). The methane gas and the liquid gasoline are separated in the drum, D-1. A heat exchanger (E-1) cools the hot liquid gasoline stream, to limit the loss of gasoline by evaporation in the drum. Valves V-4 and V-6 automatically maintain the liquid level in the drum, between set high and low limits. See the Crowl text for details of the symbols on the flowsheet diagram. ( You are encouraged to discuss this problem in “what-if” teams ) Signals (dashed lines) to and from the other valves and the pump allow remote manual control by an operator in the main control room. There are no people working near the equipment (good plant design). During a startup, the pump P-1 suddenly exploded in a large ball of fire. Although the pump and the connected piping are destroyed, the drum itself was only slightly damaged by the fire.
a. What is the most likely sequence of events that led to the destruction of the pump? List them in order of their occurrence. Hint: Use a “What-If?” analysis to consider what would happen if each of the labeled pieces of equipment did not operate as expected (either singly or together). What would cause that to happen? Safe assumptions: Pipes carrying hot gasoline would be made from metal (conductive), so static electricity is not a concern. There is no oxygen/air in the gasoline/CH4 mixture, so it cannot burn/explode unless released to the atmosphere. The pump is rated for continuous operation at the maximum temperature of the hot gasoline feed. No open flames are present.
b. Draw appropriate reliefs on the diagram, wherever they are required to prevent catastrophic failure. Err on the side of safety in your final design, but please do not simply put a relief on every pipe or piece of equipment. Copy and paste this symbol where required on the figure:

c. Describe a simple change that could be made to the piping or signal/control lines, which would completely and automatically eliminate the main cause of this explosion. Hint: Signals can be made to control equipment other than valves. Just adding a relief or two is not sufficient.
5. The photographs below show a typical underground pump vault in a petroleum refinery. The pump is used to feed a liquid hydrocarbon stream (e.g. gasoline) to a nearby distillation tower.
a) Is the pump vault a permit-required confined space? Explain in terms of the formal definitions of confined spaces. What signage would be required for this area?
b) Using the Hazard Identification Tool which was distributed in class, identify at least three potential life-threatening hazards which might exist in the pump vault.
c) List the specific items of safety equipment that are required before anyone enters the vault to perform maintenance on the pump head.
d) List and describe the role of each person who is required to be involved in the maintenance operation.
6. Refer to the process flow diagram above: Propane vapor is compressed in the turbine compressor C-1 to a final pressure of 7 atm. The boiling point of propane at 7 atm is 12 °C. The compressed vapor is then cooled with 4 °C water in the heat exchanger E-1. The cold propane vapor condenses into liquid propane, which is held in the large refrigerated tank T-1 until it is fed to a reactor. The process is controlled from a remote control room, and an overfill sensor sends an alarm signal to the operators if the liquid rises above a set level. The tank can be inerted by closing all valves except V-5 and V-7; the N2/propane vapor goes to a flare, where it is safely burnt. Valves V-7 and V-8 are normally closed. ( You are encouraged to discuss this problem in “what-if” teams)

Your arrows must be clearly drawn and must point to the part of the equipment/pipe/etc. where the relief should be installed and whether it is a vacuum or pressure relief. Points will be deducted for reliefs that are placed where they are not needed. Do not simply place a relief on every pipe and every piece of equipment!
7. Extra Credit (20pts). Read the article “Learning from UCLA” by J. N. Kemsley (https://cen.acs.org/articles/87/i31/Learning-UCLA.html). Then answer the following:
a. In 2008, a graduate student died during a “routine” organic synthesis step using a very hazardous chemical. What was this chemical and what was the primary hazard?
b. Summarize the suspected sequence of events that lead to the incident and eventual death of Sheharbano “Sheri” Sangji, the 23 year old graduate student.
c. Describe the “proper” technique to transfer the dangerous chemical. Was this technique followed exactly? Describe any differences. What was the proper PPE? Was it being worn?
d. Did regulators (Cal/OSHA) cite violations to UCLA? Why?
e. What larger, systemic, problem did this incident bring to light? Be sure your answer involves a discussion of “safety culture.”

Dialogue Project
· You will engage a member of the community outside of the classroom in a dialogue about each of the ethical topics that are the focus of this class.
· You will write paper (1,000-1,500 words) in which you describe your dialogue, draw critical conclusions about the dialogue, and defend your own thesis about the ethical topics discussed.
Late papers will be marked down by one letter grade per day late.
1. You will engage a person in a dialogue about EACH OF the applied ethical topics that are the focus of this class:
· Abortion

· Sexual Morality and Gay Marriage
· Biotechnology
· Punishment and the Death Penalty
· Violence and War
· Environmental Ethics
· Animal Welfare
2. You will write a paper that includes a report of the dialogue, an evaluation of the dialogue, and your own thesis about each of the topics
· The Paper should be minimum of 1,000 words.
· Good writing style/formatting is required: double-space, reasonable (12-point) font, 1-1.5 inch margins, etc.
· General Note: You must connect your paper to authors/ideas we’ve discussed in the class. Demonstrating knowledge of course material is essential.
The Paper should include five parts:
1. An overall thesis about ethics, the importance of dialogue, and your own thesis about ethics and the applied topics.
2. A description of the dialogue: where, who, when, etc. Explain why this person was chosen—whether they have any relevant expertise or what you hoped to learn from them.
3. An analysis/evaluation of the opinions of your partner with regard to each of the topics:
a. Do you agree or disagree with what your partner said–why?
b. What arguments could have been better–how?
c. What basic moral frameworks did your partner use or agree with?
d. Which authors would your partner agree with? Disagree with?
4. A defense of your own conclusions about each ethical topic discussed. Remember that you must discuss EACH of the topics for the course.
a. What is your thesis/argument about each topic?
b. What is your basic ethical framework (utilitarian, deontological, natural law, etc.)?
c. Which authors and ideas do you agree or disagree with?
d. How do you reply to counter-arguments/objections?
e. How does your own opinion connect with those of your dialogue partner?
5. Conclude with a summary of your own argument and what you learned from the dialogue.
Preparing for the dialogue
· Imagine who you want to talk with and arrange a specific time in advance
· Plan on at least one hour
· Let them know that this is for a class assignment
· Indicate the topics to be discussed
· Let your partner know that they can remain anonymous, if they desire
· You should prepare a list of concrete questions in advance. For example…
· Is a war against North Korea justifiable? Have recent wars in Iraq, Afghanistan, etc. produced more benefits or harms?
· Should the death penalty be abolished? Is the prison system useful? Does it deter crime or serve a retributive purpose?
· Is abortion permissible? In case of rape? In case of disabled fetus? Should Roe v. Wade be upheld or overturned?
· Is euthanasia permissible? Active? Passive? What about California’s new assisted suicide law?
· Is climate change an ethical problem? What should we do about it? What kind of value do trees, glaciers, and ecosystems have?
· Is vegetarianism morally permissible or required? Etc.
· You should plan to provide your partners with different examples, ideas, and arguments that have been discussed in class—and ask them what they think about these things. Case studies and concrete scenarios work best to stimulate discussion.
· You should plan to follow up with critical questions and discussion.
· Ask them why they think the way they do
· Ask them to imagine the implications of their ideas
· Ask them to relate their arguments to other topics and issues
· You may organize the dialogue in any fashion that works for you and your schedule.
· You may meet at all once or do it over a couple of meetings
· You may use phone, skype, or email to do the “dialogue.”
· Ideally, you should pick a partner who may have different ideas and points of view from your own; or you should pick a partner who has some important relevant expertise.
· A dialogue is more interesting when it involves a diversity of ideas.
· It is also useful to explore the thinking of family elders, clergy, authority figures…
· If there is too much agreement, you should play the “devil’s advocate”—and raise objections and arguments that disagree in order to explore ideas on the other side of things
· Be polite and courteous. You might offer to buy your partner a coffee; and you might offer to provide him/her with a copy of the paper when you are done!

Please describe how you have prepared for your intended major, including your readiness to succeed in your upper-division courses once you enroll at the university. Things to consider: How did your interest in your major develop? Do you have any experience related to your major outside the classroom — such as volunteer work, internships and employment, or participation in student organizations and activities? If you haven’t had experience in the field, consider including experience in the classroom. This may include working with faculty or doing research projects.
If you’re applying to multiple campuses with a different major at each campus, think about approaching the topic from a broader perspective, or find a common thread among the majors you’ve chosen.
Choose to answer any three of the following seven questions:
1. Describe an example of your leadership experience in which you have positively influenced others, helped resolve disputes, or contributed to group efforts over time. Things to consider: A leadership role can mean more than just a title. It can mean being a mentor to others, acting as the person in charge of a specific task, or taking lead role in organizing an event or project. Think about your accomplishments and what you learned from the experience. What were your responsibilities?
Did you lead a team? How did your experience change your perspective on leading others? Did you help to resolve an important dispute at your school, church in your community or an organization? And your leadership role doesn’t necessarily have to be limited to school activities. For example, do you help out or take care of your family? 2. Every person has a creative side, and it can be expressed in many ways: problem solving, original and innovative thinking, and artistically, to name a few. Describe how you express your creative side. Things to consider: What does creativity mean to you? Do you have a creative skill that is important to you? What have you been able to do with that skill? If you used creativity to solve a problem, what was your solution? What are the steps you took to solve the problem?
How does your creativity influence your decisions inside or outside the classroom? Does your creativity relate to your major or a future career? 3. What would you say is your greatest talent or skill? How have you developed and demonstrated that talent over time? Things to consider: If there’s a talent or skill that you’re proud of, this is the time to share it. You don’t necessarily have to be recognized or have received awards for your talent (although if you did and you want to talk about, feel free to do so). Why is this talent or skill meaningful to you?
Does the talent come naturally or have you worked hard to develop this skill or talent? Does your talent or skill allow you opportunities in or outside the classroom? If so, what are they and how do they fit into your schedule? 4. Describe how you have taken advantage of a significant educational opportunity or worked to overcome an educational barrier you have faced. Things to consider: An educational opportunity can be anything that has added value to your educational experience and better prepared you for college. For example, participation in an honors or academic enrichment program, or enrollment in an academy that’s geared toward an occupation or a major, or taking advanced courses that interest you — just to name a few.
If you choose to write about educational barriers you’ve faced, how did you overcome or strived to overcome them? What personal characteristics or skills did you call on to overcome this challenge? How did overcoming this barrier help shape who are you today? 5. Describe the most significant challenge you have faced and the steps you have taken to overcome this challenge. How has this challenge affected your academic achievement? Things to consider: A challenge could be personal, or something you have faced in your community or school. Why was the challenge significant to you? This is a good opportunity to talk about any obstacles you’ve faced and what you’ve learned from the experience. Did you have support from someone else or did you handle it alone?
If you’re currently working your way through a challenge, what are you doing now, and does that affect different aspects of your life? For example, ask yourself, “How has my life changed at home, at my school, with my friends, or with my family?” 6. What have you done to make your school or your community a better place? Things to consider: Think of community as a term that can encompass a group, team or a place – like your high school, hometown, or home. You can define community as you see fit, just make sure you talk about your role in that community. Was there a problem that you wanted to fix in your community?
Why were you inspired to act? What did you learn from your effort? How did your actions benefit others, the wider community or both? Did you work alone or with others to initiate change in your community? 7. Beyond what has already been shared in your application, what do you believe makes you stand out as a strong candidate for admissions to the University of California?
